Java Script Developer for Adobe Acrobat

We have an Adobe .pdf file that we would like to setup for our customers to choose which option of the document they would like to complete without seeing the entire document.
1) Once the recipient receives the pdf file, they will only see the first page with the options they need to choose from.
2) Once the recipient has chosen an option ONLY the appropriate pages will be shown for the recipient to complete, and the remaining pages will be left hidden from the recipient’s view.
3) Once the recipient has completed all necessary fields, we want the recipient to sign the document electronically, have an option to print the signed documents.
4) Once the recipient has signed the document electronically, we want the document to automatically be emailed back to the sender, 1 additional email user and the recipient that completed the document.
5) We would also like to have a link to this document so that we can post it on our website as well as utilizing it in Microsoft Outlook, and HubSpot.
6) Want this document to be sent by multiple people.
7) We will want you to make this document as sleek and easy to use as possible.
